APPLICATION FORM & SUBMISSION

• Availability of Forms: Application forms can be obtained from the institute's office.

• Form Fee: Rs. 500/- per form.

• Submission Dates: Forms will be received from August 24, 2025.

• Office Timings: 9:00 AM to 2:00 PM (during working days).

ADMISSION ELIGIBILITY CRITERIA

GENERAL TERMS & CONDITIONS

1. Regulatory Compliance: All admissions will be conducted in accordance with the rules and policies established by the Education Department and Health Department, Government of Punjab.

2. Incomplete Applications: Incomplete applications or those received after the prescribed date will not be accepted.

3. Merit-Based Admission: Admissions will be granted on a merit basis by the official committee.

4. Domicile Requirement (Courses A & B):

   • Admissions for Courses A and B are open to residents of all of Punjab and Azad Jammu & Kashmir (AJK), except for non-local students.

   • Residents of Azad Jammu & Kashmir are also eligible for admission to all courses.

5. Eligibility Period (Courses A & B): As per the rules of the Education Department, candidates who have passed Matriculation (Science) within the last five (05) years are eligible to apply for admission.

6. Fee & Seat Details: Detailed information regarding fees and the number of seats is attached with the admission form.

7. Special Quota for Disabled Students: A quota of 20% is reserved for differently-abled students in all courses.

8. Ineligibility for Previously Enrolled/Passed Candidates: Candidates who are already registered with or have passed the Board of Intermediate & Secondary Education (HSSC) are not eligible for Courses A and B.

REQUIRED DOCUMENTS (Attach attested copies with the application form)

*NOC / Migration Certificate is required for applicants of Courses A and B who have passed their Matriculation examination from a board other than the Lahore Board.

INTERVIEW SCHEDULE

• Date: August 31, 2025 (for Courses A & B)

Important Notes for Interviews:

• No separate call letters will be sent for interviews.

• No TA/DA (Travel Allowance/Dearness Allowance) will be admissible.

• Candidates who fail to appear on the scheduled date will not be considered eligible for admission.

• In case of a successful interview, selected candidates will be required to pay the required dues on the same day.
